We assess fall hazards, grab bars, clutter, and accessibility on every first visit, information you’d never get from a clinic visit.

We see what’s happening with medications at home: full bottles, expired prescriptions, duplicates and flag concerns to your office immediately.

Send the Referral
Fax a script or referral order to (646) 741-8768. Include name, DOB, diagnosis, and precautions. downloadable referral
We Contact the Patient
We reach out within 24–48 hours, verify insurance benefits up front, and schedule the first visit.
Evaluation & Plan of Care
We complete the home evaluation and fax the plan of care to your office for review and signature.
Ongoing Updates
We keep you informed of progress and call directly with any concerns — the same day we see them.
